In November 1996 the New South Wales Government commissioned a broad ranging Review of Mine Safety in New South Wales. The Government's decision took place in the context of continuing death and serious I injury in the state's mining industry. It also followed a number of major incidents where while mineworkers were not actually injured or killed, the potential for a major catastrophe to occur was considerable; and the release of the Warden's Court Inquiry Report into the 1994 disaster.at Queensland's Moura No.2Minein which Imen died. It was felt that the time had come for a fresh and independent look at how safety in the industry was being managed.-
